I, the undersigned, am a parent or legal guardian of the named minor. I fully understand that participating in the sport of soccer presents a risk for serious injury. In my capacity as parent or legal guardian, I understand the risks and my responsibility to notify the other parent or legal guardians as well as the minor, of the risks involved with sport participation. I have made a conscious decision to allow the named minor to play. I agree that my health and accident insurance will be the primary insurance to cover expenses for any such injury, including rehabilitation.
By checking the following statements you are agreeing to those terms:
Persons who cannot conduct themselves in the above manner are not welcome at the CSYSA soccer matches. Individuals who persist in violating these standards will be asked to leave the playing area and play will be suspended until they do so. Failure to respond to such a request within two minutes will result in a forfeiture against the offending spectator’s side.
Jersey: Jersey must be an EPYSA jersey. Jersey must be tucked in.
Shorts: Shorts must be EPYSA solid Royal blue soccer shorts.
Socks: Socks must completely cover the shin guard. Socks are a solid Royal Blue. The only time another color may be worn is if the entire team wears another color together.
Shin Guards: Mandatory for protective reasons. Must be worn to practices and games, if not the player will not be permitted to play. Shin guards must be appropriate size and cover from the full shin.
Footwear: Under 6 may wear a good pair of tennis shoes or soft-cleated soccer shoes.
All Players older than 6 must wear soft-cleated soccer shoes, no toe cleat.
Kids will not be allowed to play if toe cleat is present.
No Metal: earrings, hair clips, necklaces, as well as no watches or plastic hair bands.
Submitting this form indicates that you have read the Release Agreement and the CSYSA Standards of Conduct for Spectators and players equipment laws. The liability will lie upon the player and family and will not fall upon (EPYSA), the Recreation Department, or the Town of Bennett.
